Immunization requirements & verification process for Health Science majors

Texas law requires students involved in healthcare programs at Texas Woman’s University to show proof of certain immunizations. These students are therefore required to submit written proof of their immunizations to Student Health Services for verification. Participation in clinic rotations is contingent upon compliance with this policy, and students are advised to submit their immunization records several weeks before their clinicals so that their immunization records can be updated and reported to their departments and professors.

Immunizations should be submitted and reviewed online at TWU Patient Portal.

A demographic cover sheet is required the first time that a student submits information to the Immunization Verification Program. The demographic page for each healthcare program can be found below. Please make sure that the demographic page is signed at the bottom.

The demographic page is not required for additional submissions of records. However, please clearly label each page with the student's name and ID # or SSN.

Meningitis vaccination requirement (entering students)

Per Texas state legislation and beginning Jan. 1, 2012, all Texas college students (new or transfer) must receive a vaccination or booster against bacterial meningitis.

As of October 1, 2013, students 22 years of age or older not required to file the exemption or show proof of the vaccination.

The vaccine is required for students 21 years of age and younger of public and private colleges, living both on and off campus. An entering student is a new student or a student who has had a break of enrollment for one or more fall or spring semesters. Transfer students are considered entering students.

The student, or parent or guardian of the student, will be required to provide a certificate signed by a health practitioner or an official immunization record showing the student has received the bacterial meningitis vaccination or booster during the five-year period prior to enrollment, and not less than 10 days before the first day of classes.

The affidavit of objection form may no longer be used. Instead, applications are required to submit a notarized DSHS official Conscientious Objection Form. Please note it may take 1–2 weeks in order to complete this process. Additional information is available on the DSHS website.

Student processes to request an exemption from flu or COVID-19 vaccine requirement:

Students who are requesting an exemption from a required vaccine for a clinical or academic placement for medical or religious purposes will complete the following for review by the College of Nursing and College of Health Sciences Vaccine Exemption Review Committee.

  1. Student completes the online form. Carefully review required supporting documentation, without which review will be delayed.
  2. The committee meets on the first Wednesday of each month. Applications must be submitted a minimum of seven (7) days in advance of the meeting to be considered.

All letters of determination or requests for further information will be sent within 10 days of the scheduled full committee meeting.

The Exemptions Review Committee is not able to override any requirements from any clinical or fieldwork organizations. A letter from the Exemptions Review Committee documenting an exemption does not constitute a guarantee of clinical placement.
